The Fort Wayne Parks and Recreation Department will offer program scholarships for qualified children in 2010 as funds are available. To register for a class (excluding Zoo classes/programs) and receive one of the limited number of scholarships, please review the following:
To Qualify:
The participant must be age 18 or younger.
The participant must provide proof of income eligibility. Acceptable forms of proof are limited to:
A signed letter from the child's school stating that (s)he qualifies for the free school lunch program and free textbooks. The letter must be issued within the current school year (August through June) and should contain the child's home address. A photo copy of the letter will be considered sufficient proof.
In the case of a pre-school child, a copy of the above specified letter identifying the applicant's sibling as eligible will serve as proof.
If a letter from the school is not available, a copy of the applicant family's most recent U.S. Internal Revenue Service Tax Return form shall qualify as proof.
3.The participant may register for one activity (a class or one week of camp) up to $50 per calendar year.
4. The scholarship may be used for registration fees only. The scholarship can not be used for Zoo Camps or Zoo programs.
5. Scholarship funds can not be used towards late fees.
There will be no exceptions made to the above policy.

Fall Fun Times: August 12-November 12 To Register:
In Person: Fill out the registration form and bring it with the letter from your school stating that you qualify for the free lunch and free textbooks to: 705 E. State Blvd. Please bring a method of payment (credit card, check, cash) for additional fees over $50.
By FAX: FAX your school letter stating you qualify for the free lunch and free text books to 260.427.6020. Include your credit card number on the registration form for additional fees over $50 in case your request is denied and the entire amount will be processed on your credit card.
